Tell us the date of the violation.  

 

Let us know which of the following applies to your violation: 

• Violation is not at this property  

• Request more time to correct. (This only applies if you have received a Hearing Notice)  

• Item will be corrected per notes below  

• The violation at my property has been corrected. Please conduct an inspection to verify compliance.  

• I will attend a hearing before the Board to discuss the violation that has been reported.   

• I will not attend this meeting. However, I would like the board to review my written response below at the scheduled hearing.  

• If none of the options above apply then tell us why.
